To uncover the root cause of problems, to determine a way to do something better, or to meet a new Joint Commission on Accreditation of Healthcare Organizations requirement, staff at Methodist Hospitals of Memphis use a nine-step continuous quality improvement process.
"Once we identify a need, we use the formal process to take an intense look at the problem or issue," says Denise Thornton, RN, MSN, CDE, patient education coordinator at the health care facility. The steps help a team collect the data needed to determine a solution and create an action plan.
For example, when staff in the nursing department reviewed documentation of patient education as part of their continuous quality improvement efforts, they determined that each discipline documented in a different place, and the educational assessment was not interdisciplinary.
To remedy the situation, they assembled an interdisciplinary team that represented all clinical areas involved in teaching, such as nursing, dietary, and physical therapy. Following are a list of the steps they used to find a solution to the problem:
1. List and prioritize improvement opportunities.
Chart audits had revealed a lack of documentation of discharge planning teaching, so the team decided that improving documentation was a priority. The team also listed combining the three forms currently being used into one interdisciplinary form as a second opportunity for improvement.

Patient Surveys That Relate To Quality Improvement In Healthcare
The Centers for Medicare and Medicaid and the Agency for Healthcare Research and Quality began developing a patient survey in the early 2000s to assess care for patients admitted to hospitals. The survey was first used in 2006, and results were first publicly reported in 2008.
The survey is called the Hospital Consumer Assessment of Healthcare Providers and Systems . It is administered to patients after theyve been discharged from a hospital and asks about their experiences at the facility, including the following factors:
- How well medical providers communicated with a patient
- How responsive the staff was to a patients needs

- Whether medical providers offered important medical information to a patient when they were discharged

Leadership buy-in is essential when adopting new processes, so its crucial that external continuous improvement trainers are respectful of an organizations existing structure and its limitations therein. One thing any successful continuous improvement process needs is the ability to iterate.
